External Failure and Internal Failure Cost

Definition and Explanation of Quality Costs: The concept of Cost Of Quality (COQ) has been around for many years. Dr. Joseph M. Juran in 1951 in his Quality Control Handbook included a section on COQ. The Quality Cost Committee under the Quality Management Division was established by the American Society for Quality (ASQ) in 1961. However it was Philip B. Crosby who popularized the use of COQ because of his book Quality is Fre in 1979. Several current quality system standards, ISO 9000, QS-9000, AS-9000, reference the use of COQ for quality improvement. The concept of Cost of Quality is confusing.It does not refer to costs such as using a higher grade leather to make a wallet or using 14K gold instead of gold plating in jewelry. Instead the term quality cost refers to all of the costs that are incurred to prevent defects or that result from defects in products. What is being referenced are the costs due to the lack of quality or costs to ensure quality is produced. We understand them as the costs that are associated with preventing, detecting, and correcting defective work. Some authors refer to these costs as â€Å"Cost of Poor Quality†. Sometimes poor quality costs refer only to the â€Å"failure† costs.Crosby refers to the COQ costs as â€Å"Price of conformance† (the prevention and appraisal costs) and the â€Å"Price of non-conformance† (the failure costs). These are divided into conformance and non-conformance costs, also called control costs and failure of control costs. Figure 1 Quality costs can be broken down into four broad groups. These four groups are also termed as four (4) types of quality costs. Two of these groups are known as prevention costs and appraisal costs. These are incurred in an effort to keep defective products from falling into the hands of customers.The other two groups of costs are known as internal failure and external failure. Internal and external failure costs are incurred because defects are produc ed despite efforts to prevent them therefore these costs are also known as costs of poor quality. However, we will be focussing on the internal costs failure and the external costs failure for this assignment. The non-conformance costs come into play when the software does not conform to the quality requirements. These costs are divided into internal failure costs and external failure costs. Types of quality costs are explained below : Internal Costs Failure:Failure costs are incurred when a product fails to conform to its design specifications. Failure costs can be either internal or external. Internal failure costs result from identification of defects before they are shipped to customers. These costs include scrap, rejected products, reworking of defective units, and downtime caused by quality problem. The more effective a company's appraisal activities the greater the chance of catching defects internally and the greater the level of internal failure costs. This is the price tha t is paid to avoid incurring external failure costs, which can be devastating.On the non-conformance side, we have fault removal costs that can be attributed to the internal failure costs as well as the external failure costs. This is because if we found a fault and want to remove it, it would always result in costs no matter whether costs in an internal or external failure. Actually, there does not have to be a failure at all. Considering code inspections, faults are found and removed that have never caused a failure during testing. It is also a good example that the removal costs can be quite different due to the different techniques.When a test identifies a failure, there needs to be considerable effort spent to find the corresponding fault. During an inspection, faults are found directly. Fault removal costs also contain the costs for necessary re-testing and re-inspections. External Cost Failure: When a defective product is delivered to customer, external failure cost is the re sult. External failure costs include warranty, repairs and replacements, product recalls, liability arising from legal actions against a company, and lost sales arising from a reputation for poor quality. Such costs can decimate profits.In the past, some managers have taken the attitude, â€Å"Let's go ahead and ship everything to customers, and we'll take care of any problems under the warranty. † This attitude generally results in high external failure costs, customer ill will, and declining market share and profits. External failure costs usually give rise to another intangible cost. These intangible costs are hidden costs that involve the company's image. They can be three or four times greater than tangible costs. Missing a deadline or other quality problems can be intangible costs of quality.Internal failure costs, costs and intangible costs that impair the goodwill of the company occur due to a poor quality so these costs are also known as costs of poor quality by some persons. External failure also cause support costs. These are all costs connected to customer care, especially the effort from service workers identifying the problem. Finally, compensation costs could be part of external failure costs, if the failure caused some kind of damage at the customer site. Leadership Theory

BA365 Leadership Theory Mid Term Paper For this paper I decide to look at four of the leadership theories. The ones I chose are Trait, Contingency, Situational, and Behavioral. I will talk about the definitions of those four theories. Also I will discuss the overviews of the four theories including positives and negativities of each. Leadership Theories There is a wide and ever growing variety of theories to explain the concept and practice of leadership; to me leadership is a dynamic and complex process.Per our text book leadership is an influence relationship among leaders and followers who intend real changes and outcomes that reflect their shared purposes. (Daft, 2011) The theories that I will talk about simplify leadership into different theories. The first I will talk about is the Trait Theory. Trait theory assumes that people inherit certain qualities and traits that make them better suited to leadership. (Cherry, 2012) Also Trait theories often identify particular personality or behavioral characteristics shared by leaders. Cherry, 2012) Per our text book traits are the distinguishing personal characteristics of a leader, such as intelligence, honesty, self confidence, and appearance. (Daft, 2011) Contingency Theories of leadership focus on particular variables related to the environment that might determine which particular style of leadership is best suited for the situation. (Cherry, 2012) Success with this theory depends upon a number of variables and qualities of the followers and aspects of the situation. (Cherry, 2012) Situational Theories propose that leaders choose the best course of action based upon situational variables. Cherry, 2012) Per our book the Hersey and Blanchard’s extension of the leadership grid focusing on the characteristics of followers as the important element of the situation, and consequently, of determining effective leader behavior. (Daft, 2011) Behavioral theories leadership is based upon the belief that great lead ers are made, not born. This leadership theory focuses on the actions of leaders not on mental qualities or internal states. According to this theory, people can learn to become leaders through teaching and observation. Cherry, 2012) Trait Theory The trait model of leadership is based on the characteristics of many leaders both successful and unsuccessful and is used to predict leadership effectiveness. (Daft, 2011) The resulting lists of traits are then compared to those of potential leaders to assess their likelihood of success or failure. Scholars taking the trait approach attempted to identify physiological, demographic, personality, intellective, task-related, and social characteristics with leader emergence and leader effectiveness. â€Å"Trait Theory of Leadership†, 2012) Among the core traits identified are: achievement drive, leadership motivation, honesty and integrity, self-confidence, cognitive ability, knowledge of business, emotional maturity, charisma, creativi ty and flexibility. (â€Å"Trait Theory of Leadership†, 2012) Strengths of Trait Theory include: it’s naturally pleasing, and it gives a detailed knowledge and understanding of the leader element in the leadership process. (â€Å"Trait Theory of Leadership†, 2012) There are some limitations of the Trait Theory.They include: subjective judgment in determining who is regarded as a good or successful leader and the list of possible traits tends to be very long. (â€Å"Trait Theory of Leadership†, 2012) Contingency Theory In contingency theory of leadership, the success of the leader is a function of various contingencies in the form of subordinate, task, and/or group variables. (Daft, 2011) Fiedler’s contingency theory is the earliest and most extensively researched.Fiedler’s approach departs from trait and behavioral models by asserting that group performance is contingent on the leader’s psychological orientation and on three contextua l variables: group atmosphere, task structure, and leader’s power position. (â€Å"Contingency Theory of Leadership†, 2012) Fielder came up with his approach to leadership after realizing that leaders could function well if they changed their styles to suit the situation at hand. (Smith, 2012) According to Fiedler, leadership style may be defined as the way leaders and employees interact with one another. Smith, 2012) It should be noted that Fielder's theory does not cover all the possible factors affecting leadership. Some leaders may be more effective if they undergo training or gain experience on the job. Such factors have not been accounted for by the contingency theory. (Smith, 2012) Fielder was trying to say that leaders are not just successful or unsuccessful. Leaders can either be effective in certain situations and not all of them. Therefore, all individuals can become leaders if they choose the most appropriate situation to apply their leadership styles. Smit h, 2012) Additionally, it is possible to make a leader more effective by altering the following; position power, task structure and leader member relationships. Situational Theory The Situational Leadership Theory was developed by Paul Hersey and Ken Blanchard. The model shows that leadership of employees must be adapted to the skills and job maturity of the individual employee. (â€Å"What is the Situational†, 2010) The situational leadership style includes four leadership’s styles.They include telling, selling, participating and delegating. Also the situational theory includes four maturity levels. They include: M1, M2, M3 and M4. Telling is characterized by a strong leader categorizing roles for the employees, and who conducts his leadership with one-way communication. This leadership style is very autocratic, and is based upon followers being told what to do. (â€Å"What is the Situational†, 2010) Selling is characterized by a strong leader providing directi on, but there is more communication with followers.Leaders are trying to sell their messages to the followers, so that the followers understand the importance of their duties, and understand why different processes are important for the organization. (â€Å"What is the Situational†, 2010) Participating is characterized by two-way communication and shared decision making. Leaders include followers in job related aspects and in how task are to be accomplished. (â€Å"What is the Situational†, 2010) And Delegating is characterized by a leader leaving much of the decision making power to the followers.Leaders are still monitoring progress, but are not as heavily involved in decision making processes. (â€Å"What is the Situational†, 2010) The maturity level M1 is followers with this level of maturity lack knowledge, skills, or confidence to work on their own. They often need to be directed and supervised before they take on tasks. (â€Å"What is the Situationalâ₠¬ , 2010) Maturity level M2 is followers in this category are still unable to take independent responsibility for tasks, but they are generally willing to work at the task. â€Å"What is the Situational†, 2010) Maturity level M3 are followers in this category are very experienced and able to perform the task at hand satisfactorily. And they do not have the confidence to take on sole responsibility for task accomplishment. (â€Å"What is the Situational†, 2010) With Maturity level M4, followers are experienced, and believe that they are able to perform well. They are not only able and willing to perform their tasks, but are also willing to take on independent responsibility for the accomplishment of tasks.By using the situational leadership model, leaders honor the different needs of individuals, leaders may increase the learning curve of followers, and get a more skilled and motivated workforce in return. (â€Å"What is the Situational†, 2010) Behavioral Theory Behavioral approach does not focus on the internal states or mental qualities of the leaders. Based on this theory, a person can learn to be a leader through observation and teaching. This theory believed that the environment is a big factor of how a leader behaves. Most behaviorists assume that in order to understand the behavior of the leaders must be observed.Every behavior can be observed and monitored through the responses in every stimulus. The behavioral theory also assumes that the qualities of a leader are developed, and everyone starts with a clean slate. It is believed that the qualities of a leader are formed based also on the environmental factors. The Pyschology of Everday Life . 2000 word portfolio consisting of the following Assesment 1 - Applying psychology to ever - Essay Example In today’s material and media driven society, it has become important to have a fit and shapely body if one is to enjoy a healthy self image. Hence, currently the goal of my life is to reduce weight and to achieve a desired shape with the help of exercise and healthy diet. However, it is not as easy as it seems. After referring to different studies in the field of psychology, ayurveda and spirituality, I realized that being overweight is not just a physical problem. The tendency of overeating and accumulating fats lies in the psychological and emotional make up of a person. This means that if one has to achieve a perfect weight and a shapely body, then one has to change not only the eating habits but also the psychological aspects of the personality. Hence, to become a happier person in life, what is more important than reducing weight is to gain a healthy self image, emotional clarity and peaceful relationship with ‘self’, because one can achieve happiness only w hen one is at peace with the ‘self.’ The Root Of Problem Happiness is the most important aspect of a fulfilling life. People feel worthy and content only when they are happy with themselves. Sadly, instead of attaching happiness with the internal feelings and personality, it is related to appreciation, social success and social acceptance. Hence, people become happy only when they get appreciated and accepted by people around them. However, this leads to a major problem as it makes people depend on others for their happiness. Instead of doing things that make them happy, people start following the goals and ‘living standards’ that are set by others for them. This leads to misery and self destructive behavior pattern (Chopra, 1994, p. 42). Genuine physical beauty and emotional happiness can be achieved only when a person practices self-acceptance (Chopra, 1994, p. 42). However, self-acceptance does not come easily. The idea of ‘perfect life’ and ‘perfect body’ is shaped by the television and print media and not on the basis of self knowledge. However, it is necessary to understand that every human being is unique and hence, has a unique body and mind system (Chopra, 1994, p. 18). If a person tries to copy other people’s weight pattern or food habits, then he becomes vulnerable to losing his individuality and health. Hence, the healthier way to reduce weight is to understand the root cause of the problem and try to treat it rather than trying to reduce the weight only through exercise and diet. However, to analyze and understand the root of the problem, it is necessary to look at the problem through the psychological perspective. The problem of me being overweight is not a recent one. I have observed that this is an intermittent problem since my childhood. I have never been an obese child. However, I do have a tendency to gain weight when I go through stressful situations in life. This might be because I also tend to eat when I am tense or feel emotionally closed. This shows that it is my emotional pattern that makes me to eat unhealthy food and become overweight as I am not able to control binging on fast food. Hence, my problem is emotional based and not body based (Chopra, 2004, p.103). It has been found that for people who are emotionally dissatisfied and unhappy, ‘food’ becomes a source of satisfaction and happiness (Chopra, 2004, p.103). The Impacts Of Global Warming And Ways To Avoid Them - Research Paper Example Critics of the global warming phenomenon argue that global warming is in fact not an environmental crisis but a manufactured issue artificially created for a variety of unsavory and politically-inspired reasons. Despite the shrinking number of detractors, global warming is a phenomenon which affects our planet every single day. Seeking to address the political issues surrounding global warming, this research paper will provide a comprehensive and in-depth overview of the global warming phenomenon. We will begin with a discussion of the evidence in support of global warming and explore the connection between global warming and climate change. Although the focus of this analysis will be the political ramifications of global warming, an introductory section must explain what global warming actually is. We then explore the Kyoto Accord and discuss the detractors of this international climate agreement. Due to the politically-charged nature of this international agreement, this essay will explore whether or not there will be a â€Å"post-Kyoto† international climate agreement, and discern whether or not it would be effective at reducing carbon emissions. Although the United States has controversially not implemented Kyoto, we will explore the possible consequences to the United States of the continuation of global warming. Global warming and climate change are two terms which are often used interchangeably. Has a link between global warming and climate change been established? Yes, global warming is caused by an increase in greenhouse gases in the atmosphere. As we shall see below, the effects of global warming include changes in atmospheric temperatures and rainfall, which can lead to climate change (Arnella & Reynard 397). Global warming is most generally described as an average increase in the Earth’s temperatures.

This vast landscape is predominantly green broken only by the presence of a few cattle and a small dog all clustered relatively near the girls. For example, in Plate 11 of â€Å"Letters and notes†, Catlin sketches a man named Stu-mick-o-sucks, who was at that time the chief of the Blackfoot Nation, and in Plate 13 sketches his wife Eeh-nis-kim. Catlin makes positive comments about their ceremonial dress that they were wearing when he drew them, and states that Eeh-nis-kim had a â€Å"pleasing countenance†. He also referred to Stu-mick-o-sucks as a â€Å"dignity†, in the same way that he might refer to a foreign king of Europe (Catlin and Shippard). In addition to his sketches of Stu-mick-o-sucks, Catlin sketched other warriors and Native American tribesmen in their full ceremonial dress. The kids share their secrets, talk about various issues that concern them, discuss their perceptions about things. The dance competition event serves the perfect milieu for these young men and women to explore themselves, their dreams, their sensuality; their association with the opposite sex and to learn to coexist with harmony in a multi-cultural environment. I think it is quite relevant to any modern day society. Initially, in the movie, we see kids showing some reservation in getting together. They remain with their respective guys' and girls' groups and give out strange expressions when asked to pair up or hold hands. These kids are basically unwilling participants. With time and the consistent efforts of their dedicated teachers, the kids' inhibitions fade away, their attitudes towards their partner softens. Palliative care is supportive care which provides physical support, psychological support, spiritual support and social support and that is to provide the best as much as possible to improve quality of life (Davidson, Macdonald Newton, 2010). How can you help and support Verner from the palliative care perspective? From the case Mr. Verner has complaining from several problems related to his state of physical, psychological, social and spiritual. In the beginning I have to consider appropriate place of care either in hospital or at home if there is sufficient support in all ways (Patient UK, 2010). Then Ill start with him a comprehensive assessment for his situation from perspective of palliative care includes the physical and psychological, social, cultural and spiritual (existential). Mr. Verner has advanced heart failure or end stage heart failure where can be identified the stage according to Dunderdale, Thompson, Miles, Beer Furze (2005) by the New York Heart Association (NYHA). In addition NYHA can assess a variety of the physical symptoms and restrictions. An important aspect of Mr. Verner management is communication and listening, exploring his understanding and feelings about his illness. Exploring concerns about the future can provide opportunities to discuss death and preferences fo r end of life care (Jaarsma et al., 2009). There are physical and psychological complications caused by CHF. For example Mr. Verner case: he does not sleep at night because he has trouble breathing, probably he has pulmonary congestion/pulmonary edema because according to (American Heart Association, 2010) pulmonary edema is one of the complications of CHF. So, medical intervention is needs in order to address the symptoms experienced by the patient, because medical care is very important to reduce patient stress and anxiety. Providing support through effective communication, skills may lift the moral of the patient. During communication I have to be honest and fidelity also in dealing with this patient must be show kindness, compassion and respect. In order to helping Mr. Verner from the palliative care perspective I have to provide a good palliative symptom management, psychological, spiritual and social support will provide hope and reassurance. Emotional and social support is very important aspect for CHF patient. Where the presence of family, relatives and friends around of the patient would be a very strong supporter to improve the psychological status of the patient and reduce depression, anxiety, social isolation and loneliness (Jaarsma et al., 2009). Ill ask the provider of Social Work to communicate with family members to provide the counseling and patient needs from social services. Also the family members should be encouraged in participating with palliative care team to more improve in the physical care for the patient. Moreover, he may benefit from a referral to social services and district nursing. Liaison between his primary care team and the local palliative care team is strongly recommended and Mr. Verner could be given contact numbers for the palliative care services. Hospice care for further social support and respite may be beneficial. Providing spiritual support is one of the important aspect of palliative care whether from family or from clergy, to encourage and support the patient to let him look to the future with optimism and live with his society and daily activities in comfortable manner until he dies (Becker, 2010). Which problems and needs can you identify? Fatigue related to lack of energy. Disturbed sleep pattern related to trouble breathing. Ineffective coping related to chronic illness (Berman, Snyder, Kozier Erb, 2008). Patients needs: Information about the disease process, treatment and general advice on what to do and what not to do. Physical support and managing symptoms to relieve/reduce suffering and improve general health for live comfortably. Emotional support to reduce the psychological symptoms, where the presence of family around him will be a catalyst for this support. Social services to provide equipment such as stair lifts, ramps, commodes and information about packages of care. Enhance the care, improve quality of life and provide end life care with respect culture (customs and traditions), dignity, beneficence, sympathy and empathy. Make a nursing care plan for Verner. Explain and motivate your suggested nursing interventions in accordance with the four key areas listed in the introduction. Patient with end stage of heart failure may present with a variety of symptoms, which are similar to patients with advanced cancer (Matzo Sherman, 2010). A detailed history, physical examination, investigations and establishment of patient priorities will help in the management of their symptoms and improvement of quality of life. An accurate drug history is important due to the nature of complex drug regimens. The difficulties of coping with unwanted drug side effects may cause patients to be afraid to report their non-concordance, which may precipitate hospital admission. Common physical symptoms are fatigue, pain, breathlessness, dizziness, cachexia, anorexia, nausea, insomnia, difficulty in walking, constipation (Jaarsma et al., 2009). Communication skills are very important part in palliative care between palliative care team and patients and their families. There are small things, but significant that matter to the patient and family such as: a clean, well-pressed uniform; neat and tidy hair; an upright posture; a smile; appropriate eye contact respecting gender, age, culture or disability; a clear introduction of self and most important of all: an attitude that reflect my positive interest in them as a person (Becker, 2010). Also during communicating with the patient must repeat the information. It is possible because poor cerebral blood may lead to confusion and memory problems (Patient UK, 2010). PHYSICAL SYMPTOMS SYMPTOM CONTROL Trouble breathing, Cough in night and Sleeping disorder INTERVENTIONS (N) AND RATIONAL (R) Initial (N) Check vital signs, heart rate, blood pressure and respiratory rate depth. Observe if any wheezes and crackles in lung bases or edema. (R) This assessment will be noting and presence of fluid in the lung with change in heart and respiratory rate (Lewis et al., 2007). (N) Administer O2 and put patient on semi follower position. (R) Over volume is increased in the heart failure patient so, it results in jugular vein distention and increased hepatojugular vein also (Morton, Fontaine, Hudak, Gallo, 2005). (N) Control pain if any, discomfort feeling. (R) Patients may experience chronic pain such as oedematous limbs or osteoarthritis, or as a result of previous heart surgery (Morton et al, 2005). Ongoing Monitoring (N) Monitor vital signs, level of consciousness, oxygen saturation, cardiac rhythm, respiratory status and urinary output (Berman, 2008). Nausea, Vomiting, Decreased appetite and Lack of energy (N) Encourage the patient to eat the liquid food use a small amount of alcohol. (R) Could be good method to stimulate appetite and improving mood and general self esteem (Berman, 2008). (N) (Dehydration) Observe skin or mucous membrane dryness and edema. ( Ongoing Monitoring ) Monitor urinary output. (R) Occurs most often with CHF patients. Hypovolemia fluid shifts and nutritional deficits contribute to poor skin and edematous tissue (Morton et al, 2005). Ongoing Monitoring (N) Help patient to do daily activities such as using a wheel chair. (R) Patient needs to fully care of and need someone to help him in accomplishing daily activities at least to feel satisfied (Lewis et al., 2007). The main factors contributing to fatigue are: abnormalities in skeletal muscle due to reduced perfusion and neurohumoral changes; the side effects of medications; reduced activity; anaemia; lack of appetite and muscle wasting (Scherer et al, 2005). Fatigue causes reduced quality of life because it severely restricts patients activities and creates difficulties in walking and getting out of the house. In the end stages of heart failure even managing personal hygiene and dressing can be difficult. Fatigue can also compound other physical symptoms such as constipation, oedema and pain (Davidson et al., 2010). Access to exercise programmes may be of benefit to reduce fatigue and can give patients greater sense of well being. Explanation to the patient and his family about the physiological causes of fatigue can help them understand what they are experiencing and referral to occupational therapy of physiotherapy for advice on energy conservation and exercise can be useful. Education about healthy eating and correcting anaemia can also be beneficial (Jaarsma et al., 2009). Breathlessness Commonly caused by pulmonary oedema due to failing left ventricular function or sometimes due to anaemia. Other causes such as chest infection should not be overlooked. Anxiety, depression and inactivity can also contribute to breathlessness (Davidson et al., 2010). Increasing diuretics is the first line treatment for breathlessness due to increasing congestion and providing by Respiratory Consultant. Home oxygen may be useful for patients with daytime low blood oxygen saturations. The use of breathing and relaxation exercises can help reduce the anxiety, which often accompanies breathlessness (Davidson et al., 2010). PSYCHOLOGICAL (EMOTIONAL) SYMPTOMS Causes and effects on CHF patients Managing symptom/ Management Diagnosis of heart failure may make emotional stress. Depression, anxiety, social isolation and loneliness are common symptoms experienced by patient with end stage heart failure. The lack emotional and social support is an important predictor of morbidity and when patient become isolated and lack the ability to cope with his disease this can also be a significant predictor of mortality (Jaarsma et al., 2009). A patients experience of depression is often compounded by their physical symptoms. Psychological symptoms are can reduce quality of life. Mr. Verner says: I am not my disease, which can hinder hope for the future. From my experience when I give an opportunity to the patient as Mr. Verner condition, certainly will talk about dying. Fears of how he may die? How of pain? Emotional support is important for the patient. Effective communication with patient and his carer is needed from diagnosis and throughout the course of the illness. To maintain hope, patients can be offered good palliation of their symptoms and exploration of their preferences for care. Referral to psychology services or counselors may be required and some patients may benefit from an antidepressant (Jaarsma et al., 2009). Tricyclic antidepressants are not usually advised due to their pro-arthymic side effects. Selective serotonin reuptake inhibitor antidepressants (e.g. fluoxetine 20 mg once daily) are more commonly prescribed (Morton et al., 2005). SPIRITUAL (EXISTENTIAL) SUPPORT Spiritual support is an important aspect in palliative care. CHF reflected a gradual loss of identity and increased dependence and his illness make him incapacitate. Where it feels the burden on society and loses a sense of worth and meaning. Some patients have religious beliefs and feel comfortable than other patients who blame the Lord and say, Where is all this time? Why the God made me like this case? (Christian medical fellowship, 2011). Spiritual support is provided by a clinically certified interfaith chaplain and a qualified by the palliative care team. And chaplain role in this is to restore hope and existential then make the patient to cope the reality (University of Iowa Hospitals and Clinics, 2011). And small things will make Mr. Thomas in happiness or make a huge difference, such as to bring his cat or a visit from a close friend or inspiration in art, poetry, music (Becker, 2010).CONCLUSION Patients with CHF often experience a multitude of symptoms that affect adversely on their general health therefore it may happen to them to get sudden death. Participation with palliative care team is necessary to reduce the symptoms, provide the best as much as possible to improve quality of life and provide end life care with dignity. Nursing care plays an important role in the teamwork for patients with CHF, which can addressed with a variety of interventions, to relieve physical and psychological suffering, including treatment of pain, breathing difficulties and sleeping disorders. Communication is very important between palliative care team and patients and their families to adoption key work of care approach could improve patients access to appropriate palliative care. In addition, good communication between all those caring people for the patient in both primary and secondary care is essential. However, palliative care needs to be accessible early in the disease beginning beca use in the advanced stages patients may had worsened their health and then the team cannot provide the desired care. Finally I choose this case because I think the palliative care process as a practice in health part just for cancer patients but after dealing with Mr. Verner case I add to my nursing knowledge more specialized skills about the palliative care.

The doors could have meant many things, possibly the choices she faces daily. As she drifts back to sleep, thoughts of the dream cloud her mind. Years of research have produced findings that everyone dreams. While not everyone may remember his or her dreams, sleep studies have shown that each person does dream as he or she progresses through the stages of sleep. Whether or not these dreams contain any significant meaning for the dreamer is a source of arguments today, as well as in years past. An in depth study of dream interpretation will reveal the benefits of exploring the meanings behind dreams. To begin this study, it is helpful to first understand the different aspects of sleep. In Atkinson’s Introduction to Psychology book, she states that sleep contains five stages, including â€Å"four depths of sleep and a fifth stage, known as rapid eye movement (REM)† (193). In various sleep studies, most adults go through all stages during their normal sleeping hours. While they drift from one stage of sleep to another, activity in the brain increases and decreases. However, this study is interested mostly in the fifth stage, REM sleep. Individuals incur a great amount of details during the course of any given day, including dates, places, times and people. â€Å"During REM sleep, the brain creates a story line that allows this large volume of events to be stored and remembered in a coherent form, albeit at an unconscious level† (Chopra 107). Carlin, George 1938 -- Comedian, actor, writer. Born May 12, 1937, in the Bronx, New York. Carlin and his younger brother, Pat, were primarily raised by their mother in Manhattan’s Morningside Heights section. Mary Carlin, a devout Irish Catholic, worked as a secretary to support her children after the death of her husband in 1940. Carlin attended parochial school and much of his negative religious sentiment stems from his experience as a Roman Catholic altar boy. Carlin completed two years of high school before dropping out. At age 17, he enlisted in the U.S. Air Force as a computer mechanic and was stationed at Shreveport, Louisiana. Over the next three years, Carlin earned his high school equivalency and moonlighted as a disc jockey at a local Louisiana radio station. In 1959, Carlin teamed up with Texas newscaster, Jack Burns. The pair collaborated on a morning radio show in Fort Worth before relocating to Hollywood, where they attracted the attention of the legendary Lenny Bruce. Bruce helped Burns and Carlin secure appearances on The Tonight Show Starring Johnny Carson. Burns and Carlin eventually split up, and over the next few years Carlin continued to make numerous appearances on The Tonight Show, as well as, The Merv Griffin Show. In the early 1960s, Carlin got his start as a stand-up comic by performing on the Las Vegas circuit and entertaining TV audiences. Clarion Boys Case Essay

One problem in the Clarion School for Boys case is that employees did not have enough training on the system. On the survey taken, most employees responded that they were dissatisfied with the training they had received and most had training of only 1-3 hours. Since there was little training given on the system, the users were uncomfortable using it. It seemed that if they could not figure something out, they would give up and go back to doing it the old way. The bookkeeper commented, â€Å"I have been trying to finish this month’s books for the last two days, but I am having the same problems as last month. The accounts receivable software program is still giving me difficulties. I think I’ll just do them by hand this month.† When the system was first installed, the staff was excited about it but as time passed and there was no learning progression of the system, people became bored with it and stopped using it as much. Another comment made by the education supervisor was, â€Å"There was a lot of initial excitement about e-mail, but I haven’t heard much about it since then. I know I’ve been too busy to lean it myself, and I missed the training sessions because of other meetings.† I personally worked with a company where a new system was implemented and because of the lack of training and tutorials, there were major problems. For months the system was incorrect, causing many hours of overtime as well as many mess ups that had to be corrected. Familiar to this case, there was not a plan of action nor any goals set or a timeline for certain projects to be mastered. There was a lack of training causing the staff to have little confidence in their abilities to work the system. This problem falls under strategic planning. One component of strategic planning is training. Under strategic planning, the employees should be encouraged to be active in the system. They should be encouraged through training programs and help sessions. The team should establish goals and time lines of what they want to have accomplished by a specific date. In this case, there was not a formal plan or measures set. They were not able to judge how they were doing because they did not make a strategic plan to begin. I think this is a minor problem. I think the major problem is that once the system was implemented, there was no enforcement created. It seemed that the users did not want to get out of their comfort zone and learn something new. There was not much communication at the beginning and everyone seemed to be doing their own thing.
